Refer Yourself

Thank You!

Your referral has been submitted.

If you provided an email address you will receive an email confirmation of your referral containing the details below. Please check your junk mail and add us to your contacts/safe senders.

What happens next?

We will be in touch to offer you an initial assessment to establish your suitability for our services. Initial appointment offers are normally made by email – please monitor your email address if you have provided one.

At your initial assessment, the practitioner will ask you a series of questions to determine what is impacting your mental wellbeing. This information will allow the practitioner to consider whether we have services at FDAMH to best support your needs.

Unfortunately, in some instances we may not be the correct support and our practitioner will explain this to you.

If we can offer you support, you will be referred to one of the services at FDAMH and our practitioner will explain what to expect.

If you feel you require further assistance, you are welcome to phone us on 01324 671600.

Our office opening hours are:
Monday to Thursday: 9am to 5pm
Friday: 9am to 4pm.
Due to staff training, calls may not be answered on Wednesdays before 10.30am and Fridays after 3pm.
